This Grant Funded position is under the Ryan White program for Early Intervention Services.

 The length of employment in a grant position is subject to the continuance of grant funds, and you may be terminated without right of appeal when such funds are no longer available.
 
 
Position Summary:
Works collaboratively with the Hospital Discharge Planning Team, Emergency Department, Outpatient Care Centers, and Ambulatory Clinics to assess, educate, plan, facilitate, track, and monitor patients needing navigation into or back into HIV care. Ensure that follow up services including Ryan White services are addressed. Assists the patient and family to navigate through possible barriers to care ensuring that patients obtain medical access and referral resources in an efficient and expedited manner. Assesses knowledge about HIV, provides general information about HIV then, facilitates access to insurance, medical care and medications for patients. Works with patient and interdisciplinary team within the hospital connecting to outside agencies for services needed; including inter-facility transfers, inpatient admissions and discharges from outpatient facilities and the ED, ensuring smooth transition of care to the provider of patient’s choice.

Distinguishing Characteristics:
This is a stand-alone classification which does not fit in a classification series. 

Education/Experience:
Graduation from and accredited school of nursing and three (3) years of clinical nursing experience, two (2) years of which were in case management or providing Ryan White services. 

Licensing/Certification Requirements:
Valid License by State of Nevada to Practice as a Registered Nurse. Basic Life Support (BLS).

Knowledge, Skills, Abilities, and Physical Requirements

Knowledge of:
Nursing process (assessment, planning, implementation and coordination of patient care); standards and appropriate level of care; disease process of illnesses or injuries in variety of specialty areas; patient care plan development; patient evaluation and assessment techniques; Ryan White Early Intervention Services; Nurse Practice Act, Joint Commission on Accreditation of Health care Organizations (JCAHO), Federal and State regulations; UMC policies and procedures; department and hospital safety practices and procedures; patient rights; infection control policies and practices; handling, storage, use and disposal of hazardous materials; department and hospital emergency response policies and practices; age specific patient care practices.
 

Skill in:
Effective communication with all levels of organization; problem solving and multitasking; working with patients in a variety of conditions; interpreting rules and regulations; interpreting and analyzing patient medical charts; organizing, prioritizing and delegating work; using computers and a variety of software applications; communicating with a wide variety of people from diverse socio-economic and ethnic backgrounds under stressful conditions; establishing and maintaining effective working relationships with all personnel contacted in the course of duties; efficient, effective and safe use of equipment. 

Physical Requirements and Working Conditions:
Mobility to work in a typical clinical setting and use standard equipment, stamina to remain standing and/or walk for extended periods of time, vision to read printed materials and a VDT screen, and hearing and speech to communicate effectively in both oral and written forms. Strength and agility to exert up to 100 pounds of force occasionally, and up to 50 pounds of force frequently or constantly to lift, carry, push, pull or otherwise move objects, including the human body.
